What is Community Navigation?
Community Navigation is a free service that places experienced professionals, called Community Navigators, directly in your medical office to help guide and support you throughout pregnancy and early parenthood. From nutrition and developmental milestones to emotional wellness and educational tools, we tailor support to fit your family’s unique needs.
Think of your Navigator as your personal connection to the best resources our community has to offer. Whether you’re balancing a career, managing a busy household, or preparing for your child with intention and care, we provide resources for real life challenges including:
- Prenatal nutrition and breastfeeding support
- Childcare referrals and baby gear essentials
- Food assistance programs
- Housing resources
- Transportation support
- Mental health resources
- Parenting education
- Community resources for fathers and partners
- Milestone guidance and development checks for your child’s first 3 years
- Ongoing check-ins at 12, 24, and 36-month well-child visits
All support is provided during your routine medical visits, so there are no extra appointments required. Bilingual services are available as well.
